Please bring your insurance cards and a completed consent form. Pick one up at the Wellness office, Manor Reception, or use the fillable form at the link below.

Vaccination Consent Form

Residents may come at designated times based on last name between 10:00-2:00.

• T-Z 10:00-11:00
• N-S 11:00-12:00
• G-M 12:00-1:00
• A-F 1:00-2:00

If you do need to come at a different time, please come later during the clinic hours. We often have additional residents show up at the beginning of the clinic, causing longer wait times for the rest of the day.
